                                            Sub- request for completion of process in r/o

                                                       conversion of 5 P&T Dispensaries as CGHS

                                                       WELL NESS CENTRES-Reg

Respected Madam,

                                        It is to inform that as per the orders issued by MOH&FW  vide letter no. S.11011/01/2016/CGHS-III DT.25-1-16 the process of conversion of P&T Dispensaries has to be completed by 1-4-2019 by taking action as per the guide lines issued in the letter cited.

According to items mentioned below.

1.1. I [A] steps are to be taken to confirm staff position.

2. Item. I- Coordination committees are to be formed with two members from CGHS & two members from postal Dept for each dispensary

3. item.no.5- meetings are to be arranged with pensioners associations to sort out their problems and advisory committees are to be formed with their members

4. Item no.7- some empanelled hospitals are to be identified in those areas.

You are requested to inform the action taken on these items till date and also request you to complete the process immediately so that number of CGHS CORD holding pensioners of these areas are waiting to transfer their cords to these wellness centers and new pensioners are waiting to apply for cords.

As such you are requested to complete the process immediately and help pensioners.
